Job Description: Job OverviewAs an HAD Practitioner Doctor, you will join a specialized medical team, serving as the medical referent in a dedicated area. Your primary responsibilities include ensuring city-hospital medical coordination, promoting collaboration among various healthcare stakeholders, and directly engaging with patients through home visits or teleconsultations to ensure continuity of care.Primary Responsibilities

  • Act as the liaison between city-based and hospital-based medical teams, facilitating communication and coordination among attending physicians, hospital doctors, healthcare executives, paramedical teams, mobile units, pharmacies, laboratories, health networks, and other relevant entities.
  • Contribute to the daily operations of the division, focusing on teamwork, service enhancement, professional training, quality improvement, and practice evaluation.
  • Conduct home visits and/or teleconsultations, meeting with patients and their families, and providing necessary prescriptions to maintain continuity of care.
  • Depending on your expertise or interests, engage in the institutional life of the establishment, including participation in CME (Continuing Medical Education), other organizational bodies, transversal development, training missions, partnerships, research protocols, and innovation initiatives.

Essential Skills and Qualifications

  • Holder of a medical degree and registered with the Order of Physicians.
  • Clinical experience in either city or hospital settings is required.
  • Must possess a valid driving license (cat. B). Occasional travel for home visits.
  • Strong interpersonal and diplomatic skills, with a particular interest in improving home care pathways and enhancing city-hospital relations.
  • You are enthusiastic about integrating your actions within transversal and multidisciplinary teams.
